New Patient Form

Health History Update
To help us provide safe and personalized care, patients may be asked to complete a Health History Update. This includes changes to medications or recent surgery and treatments.
Dental Hygiene Prescription
For patients continuing care beyond 18 months, an updated prescription or authorization may be requested from their dentist, dental specialist, surgeon, or physician, as applicable.
Referring Providers
We welcome collaboration with dentists, physicians, nurses, case managers, care coordinators, senior living communities, and other healthcare professionals who are committed to improving access to preventive oral healthcare.
